The Getting Better at Chronic Care (GBACC) Project (2010-2015) involves 12 Indigenous communities across the Cairns, Cape York and Torres Strait health regions. Indigenous Health Workers (IHWs) have been recruited to work with clients in the communities in which they live. A set group of clients with diabetes and other chronic conditions were recruited to the project and are managed by IHWs using a family-centred model. On a daily basis they are monitoring current clinical needs while at the same time, educating and promoting good health behaviours amongst the wider community. The IHWs are Frank Hollingsworth (Mossman Gorge), Nanette Sands (Yarrabah), Celestine Brickey (Mareeba), Jennifer Sellick (Old Mapoon), Phyllis Wilson (Injinoo, NPA), Anna Tamwoy (Umagico, NPA), Dorelle Sambo (Seisia/New Mapoon, NPA), Debra Nona (Badu Is, Torres Strait). They all bring a substantial amount of experience to the role with many combined years of working in indigenous health in various capacities. They are supported by the Indigenous Clinical Support Team (ICST), comprising Cilla Preece who has a background as an Indigenous Health Worker and a Masters in Research and Vickie Owens, a Registered Nurse with a Masters in Public Health. Both Cilla and Vickie are assigned to support the IHWs professionally and personally as required.
